Boris Protopopov 58404a73db Refine use of zv_state_lock. ...

Use zv_state_lock to protect all members of zvol_state structure, add
relevant ASSERT()s. Take zv_suspend_lock before zv_state_lock, do not
hold zv_state_lock across suspend/resume.
