zed: mark disks as REMOVED when they are removed
ZED does not take any action for disk removal events if there is no
spare VDEV available. Added zpool_vdev_remove_wanted() in libzfs
and vdev_remove_wanted() in vdev.c to remove the VDEV through ZED
on removal event.  This means that if you are running zed and
remove a disk, it will be properly marked as REMOVED.

Fix unchecked return values and unused return values
Coverity complained about unchecked return values and unused values that
turned out to be unused return values.

Different approaches were used to handle the different cases of
unchecked return values:

* cmd/zdb/zdb.c: VERIFY0 was used in one place since the existing code
  had no error handling. An error message was printed in another to
  match the rest of the code.

* cmd/zed/agents/zfs_retire.c: We dismiss the return value with `(void)`
  because the value is expected to be potentially unset.

* cmd/zpool_influxdb/zpool_influxdb.c: We dismiss the return value with
  `(void)` because the values are expected to be potentially unset.

* cmd/ztest.c: VERIFY0 was used since we want failures if something goes
  wrong in ztest.

* module/zfs/dsl_dir.c: We dismiss the return value with `(void)`
  because there is no guarantee that the zap entry will always be there.
  For example, old pools imported readonly would not have it and we do
  not want to fail here because of that.

* module/zfs/zfs_fm.c: `fnvlist_add_*()` was used since the
  allocations sleep and thus can never fail.

* module/zfs/zvol.c: We dismiss the return value with `(void)` because
  we do not need it. This matches what is already done in the analogous
  `zfs_replay_write2()`.

* tests/zfs-tests/cmd/draid.c: We suppress one return value with
  `(void)` since the code handles errors already. The other return value
  is handled by switching to `fnvlist_lookup_uint8_array()`.

* tests/zfs-tests/cmd/file/file_fadvise.c: We add error handling.

* tests/zfs-tests/cmd/mmap_sync.c: We add error handling for munmap, but
  ignore failures on remove() with (void) since it is expected to be
  able to fail.

* tests/zfs-tests/cmd/mmapwrite.c: We add error handling.

As for unused return values, they were all in places where there was
error handling, so logic was added to handle the return values.

Upstream: Add snapshot and zvol events
For kernel to send snapshot mount/unmount events to zed.

For kernel to send symlink creates/removes on zvol plumbing.
(/dev/run/dsk/zvol/$pool/$zvol -> /dev/diskX)

If zed misses the ENODEV, all errors after are EINVAL. Treat any error
as kernel module failure.

Distributed Spare (dRAID) Feature
This patch adds a new top-level vdev type called dRAID, which stands
for Distributed parity RAID.  This pool configuration allows all dRAID
vdevs to participate when rebuilding to a distributed hot spare device.
This can substantially reduce the total time required to restore full
parity to pool with a failed device.

A dRAID pool can be created using the new top-level `draid` type.
Like `raidz`, the desired redundancy is specified after the type:
`draid[1,2,3]`.  No additional information is required to create the
pool and reasonable default values will be chosen based on the number
of child vdevs in the dRAID vdev.

    zpool create <pool> draid[1,2,3] <vdevs...>

Unlike raidz, additional optional dRAID configuration values can be
provided as part of the draid type as colon separated values. This
allows administrators to fully specify a layout for either performance
or capacity reasons.  The supported options include:

    zpool create <pool> \
        draid[<parity>][:<data>d][:<children>c][:<spares>s] \
        <vdevs...>

    - draid[parity]       - Parity level (default 1)
    - draid[:<data>d]     - Data devices per group (default 8)
    - draid[:<children>c] - Expected number of child vdevs
    - draid[:<spares>s]   - Distributed hot spares (default 0)

Abbreviated example `zpool status` output for a 68 disk dRAID pool
with two distributed spares using special allocation classes.

When adding test coverage for the new dRAID vdev type the following
options were added to the ztest command.  These options are leverages
by zloop.sh to test a wide range of dRAID configurations.

    -K draid|raidz|random - kind of RAID to test
    -D <value>            - dRAID data drives per group
    -S <value>            - dRAID distributed hot spares
    -R <value>            - RAID parity (raidz or dRAID)

The zpool_create, zpool_import, redundancy, replacement and fault
test groups have all been updated provide test coverage for the
dRAID feature.

ZED: Do not offline a missing device if no spare is available
Due to commit d48091d a removed device is now explicitly offlined by
the ZED if no spare is available, rather than the letting ZFS detect
it as UNAVAIL. This broke auto-replacing of whole-disk devices, as
described in issue #10577.  In short, when a new device is reinserted
in the same slot, the ZED will try to ONLINE it without letting ZFS
recreate the necessary partition table.

This change simply avoids setting the device OFFLINE when removed if
no spare is available (or if spare_on_remove is false).  This change
has been left minimal to allow it to be backported to 0.8.x release.
The auto_offline_001_pos ZTS test has been updated accordingly.

Some follow up work is planned to update the ZED so it transitions
the vdev to a REMOVED state.  This is a state which has always
existed but there is no current interface the ZED can use to
accomplish this.  Therefore it's being left to a follow up PR.

Add device rebuild feature
The device_rebuild feature enables sequential reconstruction when
resilvering.  Mirror vdevs can be rebuilt in LBA order which may
more quickly restore redundancy depending on the pools average block
size, overall fragmentation and the performance characteristics
of the devices.  However, block checksums cannot be verified
as part of the rebuild thus a scrub is automatically started after
the sequential resilver completes.

The new '-s' option has been added to the `zpool attach` and
`zpool replace` command to request sequential reconstruction
instead of healing reconstruction when resilvering.

    zpool attach -s <pool> <existing vdev> <new vdev>
    zpool replace -s <pool> <old vdev> <new vdev>

The `zpool status` output has been updated to report the progress
of sequential resilvering in the same way as healing resilvering.
The one notable difference is that multiple sequential resilvers
may be in progress as long as they're operating on different
top-level vdevs.

The `zpool wait -t resilver` command was extended to wait on
sequential resilvers.  From this perspective they are no different
than healing resilvers.

Sequential resilvers cannot be supported for RAIDZ, but are
compatible with the dRAID feature being developed.

As part of this change the resilver_restart_* tests were moved
in to the functional/replacement directory.  Additionally, the
replacement tests were renamed and extended to verify both
resilvering and rebuilding.

d48091de81 zed: detect and offline physically removed devices
This commit adds a new test case to the ZFS Test Suite to verify ZED
can detect when a device is physically removed from a running system:
the device will be offlined if a spare is not available in the pool.

We implement this by using the existing libudev functionality and
without relying solely on the FM kernel module capabilities which have
been observed to be unreliable with some kernels.

* Teach ZED to handle spares usingi the configured ashift: if the zpool
   'ashift' property is set then ZED should use its value when kicking
   in a hotspare; with this change 512e disks can be used as spares
   for VDEVs that were created with ashift=9, even if ZFS natively
   detects them as 4K block devices.

 * Introduce an additional auto_spare test case which verifies that in
   the face of multiple device failures an appropiate number of spares
   are kicked in.

 * Fix zed_stop() in "libtest.shlib" which did not correctly wait the
   target pid.

 * Fix ZED crashing on startup caused by a race condition in libzfs
   when used in multi-threaded context.

 * Convert ZED over to using the tpool library which is already present
   in the Illumos FMA code.

b25c5b0639 Fix undefined reference to `libzfs_fru_compare'
Add trivial libzfs_fru_compare() function which can be used when
HAVE_LIBTOPO is not defined.  The only caller is find_vdev() and
this function should never be reached because search_fru must be
NULL unless HAVE_LIBTOPO is defined.

Rename _HAS_FMD_TOPO to existing HAVE_LIBTOPO which was
originally added for this purpose.  This macro will never be defined.

976246fadd Add illumos FMD ZFS logic to ZED -- phase 2
The phase 2 work primarily entails the Diagnosis Engine and
the Retire Agent modules. It also includes infrastructure
to support a crude FMD environment to host these modules.

The Diagnosis Engine consumes I/O and checksum ereports and
feeds them into a SERD engine which will generate a corres-
ponding fault diagnosis when the SERD engine fires. All the
diagnosis state data is collected into cases, one case per
vdev being tracked.

The Retire Agent responds to diagnosed faults by isolating
the faulty VDEV. It will notify the ZFS kernel module of
the new VDEV state (degraded or faulted). This agent is
also responsible for managing hot spares across pools.
When it encounters a device fault or a device removal it
replaces the device with an appropriate spare if available.

d02ca37979 Bring over illumos ZFS FMA logic -- phase 1
This first phase brings over the ZFS SLM module, zfs_mod.c, to handle
auto operations in response to disk events. Disk event monitoring is
provided from libudev and generates the expected payload schema for
zfs_mod. This work leverages the recently added devid and phys_path
strings in the vdev label.
