Updated AUTHORS, COPYING, DISCLAIMER, and INSTALL files. Added
standardized headers to all source file to clearly indicate the
copyright, license, and to give credit where credit is due.

fleshed out a few stub headers,
added a FIXME file,
added various compatibility macros
